Derbyshire NHS Foundation Trust is looking to recruit an enthusiastic and compassionate Nurse to join the Dementia Rapid Response Team; an innovative service, based at Albany House, Kingsway Site, Derby.

An exciting opportunity has arisen for a Lead Nurse with post-qualification experience, within the Dementia Rapid Response South Derbyshire and City team.

This well-established, multi-disciplinary team offers, a high-quality service . This includes assessment and home treatment as an alternative to hospital admission, to individuals experiencing acute mental health crisis.

We work in partnership with clients, families and existing service providers and link in with other services.

We also act as gatekeepers where inpatient, psychiatric admission is indicated.

Main duties of the job

You will be able to sustain a respectful, consistent and reliable therapeutic relationship with service users/carers/families and work in a short-term, solution focussed way.

Service users may have a range of difficulties including mental health, social, psychological and situational and the post holder will focus on enhancing mental well-being and reduce distress.

You will have excellent triaging skills and offer support and guidance to all who contact us and be able to work as lead professional for our service users during their time with us as well as working together within the team.

You will be a role model for others; expected to supervise students (with training provided where necessary) and may be a lead in a certain specialism/area.

You will work as duty worker for the team at times and work well under your own initiative as well as within the team, being able to conduct visits in patients own homes or in care homes within South Derbyshire and City area.

The service consists of a multi-disciplinary team working with older adults with a diagnosis of Dementia.

The service acts as an alternative to hospital, facilitating early discharges from the acute setting and preventing hospital admissions where ever possible and appropriate.

The team will work with a shared caseload; providing short term, intensive interventions.

We are looking for an experienced, mental health nurse to be part of this excellent service, someone who is focused on making a positive difference to peoples lives.

As a Lead Nurse, there is an expectation that you will be able to make complex decisions, lead a shift, cover duty and formulate detailed assessments and person-centred care plans to assist in the prevention of hospital admissions where appropriate.

  • Personal development will be supported and encouraged through Appraisal, regular supervision and access to training opportunities.
  • The post holder will be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.
  • The post holder will be expected to travel independently across the catchment area of the service and occasionally outside of this if required.
  • The service operates 7 days per week, the post holder will work a shift pattern facilitating this which includes working from 8am-8pm, Mon-Fri and 9-5pm at weekends/Bank Holidays.
  • The post holder will undertake the Duty role; triaging referrals, allocating visits, preparing and delivering handover, ensuring colleagues are "safe and well".
  • MDT working; liaising with a multitude of different professionals and agencies.
  • Completing initial assessments of patients.
  • Applying knowledge of the Mental Capacity Act and Mental Health Act when required.
  • Assessing risk and formulating safety plans accordingly.
  • Formulating person centred care plans focusing on non-pharmacological interventions.
  • Visiting patients in their own home and those in 24 care.
