The Older Adult CMHT for Killamarsh and North Derbyshire have an opening for a Band 6 Lead Nurse to join the team.

We are looking for an individual who has an interest in the specialist area of older adults, who strives to provide person-centred and holistic care.

As a Lead Nurse within the CMHT you will have the opportunity to support those with functional and organic presentations.

The role involves managing a caseload of patients, whilst having the opportunity to be part of the teams Care Homes Project and join the local GP CST meetings.

Excellent communication, assessment, risk screening and care planning skills are required.

Development is actively encouraged and there is access to regular supervision, as well as an annual appraisal.

Main duties of the job

The main duties of the job include:

- Complex caseload management.

- Assessment, risk screening, care planning and use of a range of assessment tools such as ACE-III, DemQol, ReQol, HoNOS.

- Administration of depot injections.

- Undertaking the role of Duty Worker, triaging referrals into the team.

- Supervising junior member of the team.

- Supervising and assessing student nurses.

- Working with the MDT, including chairing CAS meetings.

- Identifying safeguarding issues and acting on these appropriately.

- Working within the guidance of local and national policy.
