Job summary
Over the past few years, we have expanded our mental health pharmacy team in Derbyshire significantly,to enable us to extend our service and make a meaningful difference to patients in the community, while maintaining our respected service to inpatient areas.

We now have an exciting opportunity for two pharmacists to join our established and respected clinical pharmacy team.
These posts will be working within our inpatient teams, providing a clinical pharmacy service to acute adult inpatients, older adult inpatients, adult rehab, adult forensic inpatients and crisis teams.
They are rotational posts, to allow the post-holder to gain experience of providing a clinical pharmacy service in a variety of different settings.
Our pharmacists are provided with opportunities for personal growth and development, including full funding for post-graduate qualifications in mental health pharmacy, including the certificate and diploma, and independent prescribing qualification.
The department also provides trainee pharmacist foundation training, in house revalidation support and funding for conference attendance.

Main duties of the job
The roles and duties of this posts include:
- Developing close working relationships with other members of the multi-disciplinary team and contributing to clinical care plans.
- Responding to medication-related queries, providing advice on prescribing and promoting evidence-based practice.
- Discussing medication with patients and carers and developing a variety of approaches to support informed care in the community.
- Educating colleagues about medication management and medicines optimisation.
- Participate in the pharmacist rotas for Bank Holidays, out-of-hours ("on-call"), early/late cover and extra duties in accordance with contracted terms and conditions.

- Take local responsibility for pharmacist services to clinical areas in line with rotas.
- Review prescriptions and information relating to individual patients and provide relevant advice about medicines, possible side-effects, dosages, administration, supply, storage and destruction.

- Provide specialist psychiatric medicines-related advice to healthcare professionals, including the completion of detailed medication histories; with interpretation, comparison of options and recommendations for treatment and/or monitoring.
- Review new medicines and new indications for existing medicines as directed by senior pharmacy staff
- Contribute to the departments development and review of medicines-related policies and guidelines
- Develop, participate in, evaluate and present audit of pharmacy activity or medicines management / clinical audit.
- Attend external meetings (including conferences) as required, in order to share good practice, present audit / research / service development work and raise the profile of the department and Trust.
