The Dynamic Hospital Avoidance Lead (Acute inpatients & AMH Crisis/ Liaison , is an exciting role which aims to make a big impact in people's lives with Learning Disabilities and/or Autism.
The post holder will support Crisis Resolution & Home Treatment Teams and the Mental Health Hospital Liaison teams with people on their caseloads who have a diagnosis of Learning Disability and/or Autism. The main aim of the role is to support teams to avoid clinically inappropriate admissions into a mental health acute hospital.
At times someone will need to be admitted to hospital and those admissions should be as short as possible; therefore they will need to support inpatient teams to ensure robust care and discharge planning is in place for individuals with a diagnosis of a Learning Disability and/or Autism.The role will have a specific focus to provide additional support to wards to make things happen as required to ensure discharge plans remain on track. This will include liaising and working with colleagues from across Health and Social Care systems.Maintain oversight of the persons discharge planning and provide regular feedback to CCG as part of weekly Discharge Review Meeting.

This is an exciting opportunity to be part of a specialist team and contribute to an integrated service development.
The Dynamic Hospital Avoidance Lead (Acute Inpatients & AMH Crisis/ Liaison) will be to work across the mental health crisis teams and mental health hospital liaison covering the North of Derbyshire County) to support people on their caseload with adults who have a diagnosis of a Learning Disability and/or Autism. The role will be to support teams to avoid an inpatient admission if this is deemed clinically inappropriate.
At times someone will need to be admitted to hospital and those admissions should be as short as possible; therefore they will need to support inpatient teams to ensure robust care and discharge planning is in place for individuals with a diagnosis of a Learning Disability and/or Autism.

- The Transforming Care Programme (TCP) has a focus on improving health and care services so that people with Learning Disabilities and/or Autism can live in the community, with the right support, and close to home. Sometimes someone will need to be admitted to hospital and those admissions should be as short as possible. The post holder will:
- Develop close working relationships with consultant psychiatrist/inpatient teams, the TCP, Learning Disability & Autism Leads and Autism Leads) and social care to ensure timely discharges if an inpatient and to ensure that community teams are working towards the Building The Right Support Agenda.
- Work from within the Derbyshire Healthcare NHS Foundation Trust (DHcFT) Intensive Support Teams (IST) and Specialist Autism Teams (SAT) and in-reach into DHcFT acute inpatient units and Acute Mental Health community services.
- The role aims to provide support to staff within the inpatient services and Adult Mental Health community services via clinical leadership in relation to individuals with a diagnosis of Learning Disability and/or Autism.
- Provide highly specialist Learning Disability & Autism Spectrum Disorder advice and consultation to other professionals across Adult Mental Health inpatient wards, Crisis Resolution & Home Treatment Teams and Hospital Liaison.
- To have highly skilled clinician who will support and advise psychiatrist/inpatient teams and acute AMH community teams to ensure robust care and discharge planning is in place for individuals with a diagnosis of a Learning Disability and/or Autism.
