CYP Complex Case Strategic Lead
AfC Band 8a
37.5 hours per week. Some flexibility can be offered.
An opportunity has arisen for a new role, which has been developed to help support health and social care providers in Derbyshire.
Working to support existing care teams & care co-ordinators, you will help where young people present with complex emotional and social needs, and help to ensure they receive timely, supportive, person centred and least restrictive care.
Hosted by Derbyshire Healthcare NHS FT, you will work with our CAMHS teams, with University Hospitals Derby & Burton NHS FT, Chesterfield Royal NHS FT Children's teams, CAMHS inpatient units, Social care colleagues and also Commissioners.
Main duties of the job
As well as helping to develop and support individual packages of care, you will help design and make recommendations for future care provision.
You will need to have experience of working with young people, navigating a complex care landscape and a real desire to make a difference to families and young people.

Job responsibilities
Work with individual cases:
- Oversee the acute paediatric inpatient case load (at Royal Derby and Chesterfield) of individuals with highly complex mental health and /or/ learning disability and /or autism needs, or provide key support and ability to drive action and advocate for the young person to ensure prompt and safe discharge plan
- Ensure that all assessments are holistic in nature and are focused on understanding and meeting need, taking into account the young persons thoughts and wishes.
- Ensure continuity of care throughout the period of treatment until discharged back to community support. This and smooth onward transition.
- Liaise and lead planning with community services for particular complex service users.
- To identify barriers and service deficits that may hinder or prevent a person being discharged to community and report / escalate these barriers and deficits to commissioners for joint resolution.
- Prioritise and attend meetings that enable and facilitate safe and efficient discharge from acute hospital beds. Support existing care staff and co-ordinators to plan and deliver are plans and packages in complex situations.
- Liaise with multiple professionals involved in the patients care to ensure that a comprehensive understanding of all identified needs is available. Ensuring that each patient has a care plan, an identified care coordinator and where necessary a supervising psychiatrist in line with Department of Health Policy. Co-originate the multiagency meetings where needed, often at pace.
- Identify and process any safeguarding and quality of care issues and take appropriate action to ensure that young peoples welfare is protected and that the quality and standard of services provided are at and beyond levels that are anticipated.
- Develop and maintain effective working relationships with acute clinicians, general practitioners, social care staff and providers in order to ensure continuity of care and address patient needs.
- Liaising with a variety of providers and arranging assessment of patients in order to ascertain the most appropriate placement to be identified.
- Provide information and support to the commissioners when considering the most appropriate provider to address a patients identified needs.
- To maintain a formal record of all cases and ensure that all relevant clinical information is available to commissioners and updated regularly.
- To call and lead multi-disciplinary meetings, including case conferences and discharge meetings to ensure continuity of care across agencies; thus improving patient and family experience.
