Job summary
An exciting opportunity has arisen for a Band 5 Registered Mental Health Nurse to join the Older Adult South Dales CMHT.
The team is based at St Oswald's Hospital in Ashbourne. A valid driving licence and access to suitable transport is essential.
The team is small, friendly and multi-disciplinary in nature.
The role involves supporting both organic and functional patients in their own homes or 24-four hour care settings.
This is a great development opportunity for an individual who wants to work in CMHT.
All team members have access to clinical and management supervision, along with the opportunity to attend training.
Main duties of the job
Main duties of the job include:
- Assessment and care planning.
- Case load management
- Working and supporting carers
- Delivering treatment and intervention
- Attendance at MDT
- Administering depots
- Supporting the care home project
- Supporting junior members of staff and students

- Utilise best evidence in order to analyse and to undertake a core mental health assessment, formulate, plan, implement and evaluate care for a range of patients referred within the CMHT, as part of an effective multi-disciplinary approach who have specialist clinical conditions.
- Communicate complex, and sensitive information in circumstances where mental ill-health creates barriers to understanding, and to overcome barriers to establishing good relationships to enable care planning and collaborative working.
- Be responsible for assuring individuals on caseload have a personalised plan of care that is formulated in a collaborative way and with agreed outcomes associated with current evidence.
- Assess and devise care plans and deliver care in accordance with the most recent evidence-based guidance and research.
- Assume the role as a lead professional/Care Co-ordinator for patients in their caseload.
- Be responsible for the delivery of core interventions and Wellness Recovery Action Plans (WRAP).
- Ensure that patients on their caseload have up to date safety plans in place, completed in collaboration and shared where appropriate across the health community.
- Participate in the process of clinical supervision, both as supervisor, where delegated and supervisee.
