Job summary

An exciting opportunity has arisen for a Band 6 Mental Health Nurse to join the Older Adult In-reach and Home Treatment Team, based in South Derbyshire/City.

The primary goal of the service is to support older adults within their own homes who are experiencing a crisis in their mental health, to prevent hospital admission where possible, and in-reach to those already in hospital, facilitating early discharge.

The IRHTT takes a person centred approach to support those living with complex functional mental illness in the community.

The team includes a multitude of different professionals to ensure a holistic experience for the service user, including Occupational Therapists, Specialist Support Workers, Nurses, a Psychologist and a Consultant Psychiatrist.

Main duties of the job

  • The post holder will be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.
  • As a senior registered practitioner they are professionally and legally accountable under the NMC code of professional practice.
  • MDT working, liaising with a multitude of different professionals and agencies, in particular Primary Care and Adult Social Care
  • Completing initial assessments when patients are referred into the service
  • Assessing risk and formulating safety plans accordingly
  • Formulating person centred and holistic care plans with a focus on non-pharmoclogical interventions
  • In-reaching to inpatient wards and visiting patients residing in 24hour care settings
  • Putting into practise the principles of the Mental Capacity Act and Mental Health where appropriate
  • Working closely with carers and relatives to offer support and improve the provision of care for the service user
  • Undertaking the Duty role, triaging referrals, allocating visits, preparing and delivering handovers, ensuring staff are "safe and well"
  • Using the EPR to ensure care is documented

The successful applicant will work within a supportive MDT and have close links with the Community Mental Health Teams, GPs and social care.

We are also co-located with our established, Dementia Rapid Response Team.

Professional development and further learning is encouraged and you will receive robust clinical and managerial supervision, along with wider support and training in-house within the team.

You will also be expected to provide supervision to team members.

The team currently works a shift pattern covering, Monday-Friday, 8.00am-8.00pm; and 9am - 5pm Saturdays, Sundays and Bank Holidays on a rota basis.

The team covers 365 days a year, the ability to work shifts is essential.
