The Older Adult In-reach and Home Treatment Team is seeking an enthusiastic and compassionate individual to join us in Derbyshire.

The team base is at Albany House, situated within the picturesque grounds of Kingsway Hospital, with close links to the A38 and M1.

The candidate will be required to work 9-5, across Monday-Friday each week.

This service works with Older Adults (age 65+) who are experiencing a crisis in relation to a functional mental health condition.

The primary goal of the IRHTT is to support service users within the community (both in their own homes and in 24 hour care), to prevent hospital admission where possible.

In addition, the IRHTT regularly attends in-patient wards to work with service users who are close to discharge, to reduce their length of stay in hospital.

Main duties of the job

You must have a full UK Driving License and the means to travel around the community.

Assess patients for Occupational Therapy needs, formulate and deliver person centred care relating to this

Supervise and guide the team Occupational Therapy Assistant in implementing OT care plans

You will be able to make complex decisions, lead a shift, cover duty, formulate detailed assessments and person-centred care plans to assist in the prevention of hospital admissions.

Comprehensive risk assessing and care planning.

Experience of working with functional mental health illness is essential.

The successful applicant will work within a supportive MDT.

Professional development, learning, supervision, wider support and training is provided.

The service operates 7 days per week, 365 days a year. The working hours are 9am-5pm. The post holder will be expected to work some weekends and bank holidays.
