Job summary

The Transfer of Care Hub at Blackpool Teaching Hospitals NHS Foundation Trust are delighted to offer an exciting opportunity and invite applications for experienced, highly enthusiastic individuals to join our team.

The Transfer of Care Hub (TOCH) provides hospital based staff a central route to refer and access community based services to support and facilitate patient discharges.

Main duties of the job

As a Band 4 Administrator you will provide support to the Transfer of Care Hub to provide a high quality secretarial, administration and information service and act as a central reference point to all staff within the service including clinicians, managerial staff and Trust stakeholders.

The role comprises of a wide range of duties and requires the ability to use multiple computer systems, be able to manage and prioritise own workloads without direct supervision and have strong communication and organisational skills. An ability to understand the need for maintaining confidentiality at all times is essential.

The post will include weekend working and Bank Holiday cover on a rota basis.

This is an exciting opportunity to join an evolving service to ensure patients receive enhanced discharge planning, continuity of care and a seamless transition between care providers beyond the acute hospital environment.
