Job summary
An exciting opportunity has arisen within the integrated South Neighbourhood Team for a Band 6 Physiotherapist.
The team are responsible for providing assessment and rehabilitation to patients in the Blackpool community in their own homes.
As a member of an integrated team you will be supported by a friendly team of experienced and dynamic Occupational Therapists, Physiotherapists, district nurses, matrons, case managers, health and wellbeing workers and neighbourhood practitioners.
The team work closely with Social Care and care providers to assess and provide treatment and equipment for patients. As part of this role you will be offered regular supervision and gain experience from MDT working.
This role is to be offered as a permanent contract based with The South Neighbourhood Team.
The post involves delivering therapy in patients homes therefore use of a car during the day is essential for this role.

Main duties of the job
- Providing clinical physiotherapy support to patients in their own home who are experiencing physical difficulties at home.
- Ensure the smooth and efficient running of the Central West Therapy Service as the senior professional.
- Assessment of patients: plans, implement, evaluate and reassesses care of patients in their own home.
- To be responsible for assessing referrals to ensure they meet the clinical criteria for the South Community Therapy Team.
- Provide physiotherapy expertise to maintain independence of patients in their own home.
- Liaise with other services within health and social services, voluntary and private.
- Maintain excellent communication with our Partners,
- Provide high level assessment skills and direct patient care.
- Contribute to the development of the South Therapy Service.
- Support community schedule for students and Junior staff: provide induction training and clinical supervision.
- Possess a positive can do attitude.
- Support Junior staff to achieve relevant competencies relevant to specific job roles.
