An exciting opportunity to join the Blackpool teaching hospitals patient flow team has arisen. This post is for a band 5 working 21 hours per week to cover primarily twilight shifts (7.30pm-2am) but the successful candidate may be required to work some long days in this role.
Candidates must be able to work as part of a team, with an organised approach to work, paying attention to detail and prioritising their workload. The post holder must be confident in using electronic systems, ideally having experience in patient flow. This role will involve some office based work and visiting the wards throughout the trust. The successful candidate must have excellent communication skills and confident in dealing with queries.
The Bed Managers have a key role in the management of each patients' length of stay; Facilitating discharges and supporting the delivery of the A&E clinical quality indicators through the timely delivery of beds for AMU, ED, SAU and all alternative admission pathways to achieve flow and optimum placement of patients. Monitor discharge lounge usage, assist with booking transport, facilitate repatriation of patients both in and out of the organisation.
The role will require the individual to be familiar with Microsoft Office, Excel and Emails and all internal IT systems used to assist with flow and patient management.
Main duties of the job

Key duties and Responsibilities
- To visit all in patient areas to understand the bed status and the discharge plans for the day.
- Effective bed allocation whilst on twilight shifts.
- Develop effective working relationships based on trust and co-operation
- Discuss and be involved in the assessment of patients suitability of being in sidewards and ensure they are moved out in a timely manner to create further ICP capacity.
- Assessment of the placement of patients across the organisation and ensure they are moved and accommodated in the most appropriate area related to the patients clinical need.
- Utilise the information contained within the Patient Flow Tracker (NPR) to assist with discharges and aid in the unblocking of system delays.
- Identify patients whose discharge can be expedited earlier in the day.
- To support clinical areas in the delivery of timely, effective, safe discharges.
- Work with colleagues to expedite investigation, obtain senior medical reviews, involve TOCH in patient management.
- To receive and action promptly the request for bed allocation for routine, planned, urgent and emergency admissions daily.
- To assist the Clinical site Matron in identifying Golden Patients and continue to establish a process for delivering an early morning discharge, supporting the wards
- To book transport and complete TTOs for the following day.
- Inform the AMU coordinator of available beds on the downstream wards as soon as they become available using the IT systems, enabling the coordinator to allocate patients appropriately and then allocating patients on the Base tracker.

- To communicate with discharge facilitators on the wards to establish query discharges and the number of definite discharges, with an emphasis on the morning discharges.
- To collect transport booking requests from wards to ensure timely and appropriate transport is booked out of hours.
