Job summary
An exciting opportunity has become violable within the Integrated Community Learning Disability Team, and we are seeking to recruit an individual who is passionate about supporting individuals with a diagnosis of learning disabilities.
Main duties of the job
We are looking for the successful candidate to be able to:
- assess, plan, implement and evaluate care for a defined caseload
- ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team
- work in partnership with the primary health care team and liaise and cooperate with everyone involved in holistic patient care.
- be able to demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients
- Contribute to the management and leadership of the nursing team on a daily basis.
- To undertake screening assessments in determining eligibility for the service and ensure any unmet or ineligible needs are forwarded to the relevant agency
