Job responsibilities

• To work as part of the Integrated Learning Disability Team providing intensive support to people with a learning disability who have complex physical and emotional health needs.
• To support the Lead Learning Disability Nurse in planning, leading and delivering intensive support to people with a learning disability who have complex physical and emotional health needs.
• To promote and facilitate access to a range of health services by acting as a key point of contact and working to improve care delivery across the health and social care economy.
• Provide clinical supervision to junior staff and support staff development

Qualifications

Essential

• Registered Learning Disability Nurse
• Current Professional Registration
• Documented evidence of continuing professional development
• ENB 998 or equivalent mentor qualification

Desirable

• Evidence of Post registration training relevant to this post e.g. Specialist Practitioner, NMP
• Bsc Specialist Practitioner

Experience

Essential

• A minimum of one year post qualification in practice within a community learning disability team.
• A minimum of 2 years post qualification in practice within a Community learning disabilities or complex needs team.
• Experience of complex health assessments.
• Experience of working in a community multi-disciplinary team
• Ability to develop risk assessment and management plans
• Experience of care co-ordination of people with complex health and social care needs.
• Evidence of leading or contributing to service development
• Experience of caseload management.
• Experience of supervising junior or support staff
• Excellent assessment and report writing skills.
• A good understanding of the mental health needs of people with a learning disability
• Critical thinker/ability to analyse
• Knowledge of current legislation and policies for people with LD and complex health needs

Desirable

• Able to lead and facilitate meetings/ training events
• Teaching skills
