Job summary

An exciting opportunity has become violable within the Integrated Community Learning Disability Team, and we are seeking to recruit an individual who is passionate about supporting individuals with a diagnosis of learning disabilities.

Main duties of the job

We are looking for the successful candidate to be able to:

  • assess, plan, implement and evaluate care for a defined caseload
  • ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team
  • work in partnership with the primary health care team and liaise and cooperate with everyone involved in holistic patient care.
  • be able to demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients
  • Contribute to the management and leadership of the nursing team on a daily basis.
  • To undertake screening assessments in determining eligibility for the service and ensure any unmet or ineligible needs are forwarded to the relevant agency

  • Current Professional Registration
  • Documented evidence of continuing professional development
  • ENB 998 or equivalent mentor qualification

Desirable

  • Evidence of Post registration training relevant to this post e.g. Specialist Practitioner, NMP
  • Bsc Specialist Practitioner

Experience

Essential

  • A minimum of one year post qualification in practice within a community learning disability team.
  • A minimum of 2 years post qualification in practice within a Community learning disabilities or complex needs team.
  • Experience of complex health assessments.
  • Experience of working in a community multi-disciplinary team
  • Ability to develop risk assessment and management plans
  • Experience of care co-ordination of people with complex health and social care needs.
  • Evidence of leading or contributing to service development
  • Experience of caseload management.
