Job summary

An exciting opportunity has arisen for a permanent part time post for 18 Hours for the WREN locality (Kirkham, Great Eccleston, Garstang and Over Wyre areas) as a Health and Wellbeing Support Worker based within the supportive and enthusiastic Fylde Enhanced Primary Care Team. We are an integrated neighbourhood care team consisting of Community Therapists, Community Matron team, Care home team, Integrated Care Coordinators, Health and Wellbeing Support workers and Neighbourhood Assistants working also with the District Nurses.

Our Service Aim is to:

'Engage patients to improve their abilities, knowledge, skills and confidence working towards the better management and improvement of their health conditions and therefore their well being'

We pride ourselves on our relationships within the LSA and WREN neighbourhood and with other statutory and voluntary agencies. You will be supported and encouraged to use all your skills to allow yourself, the team and your patients to flourish.

Main duties of the job

We are looking for a caring and motivated individual to join our neighbourhood care teams to support patients using coaching techniques to better manage their own long term health conditions and wellbeing.

The successful post holder will work with others in the team. The aim of the role is to support patients within their own homes and also within care homes in order to make lifestyle and behaviour changes to improve and self-manage their health and wellbeing.

The role will also involve group work delivering our successful wellbeing programme and can also include clinically related tasks that the patient may require to support this role.

  • Level 3 NVQ Health/ Social Care
  • Care Certificate or working towards (or ability to complete)

  • Coaching qualification

Experience

Essential

  • Evidence of relevant experience
  • Basic nursing and or therapy skills
  • Experience of undertaking clinical skills such as basic dressing, routine monitoring of patients observations, venepuncture, catheter care etc. or willingness to undergo training to do this
  • Demonstrate IT literacy and competency with e-mail, word, intranet etc.
  • Experience of coaching or counselling regarding life skills

Desirable

  • Evidence of successful completion of relevant recent training in e.g. equality and diversity, disability awareness, safeguarding, moving and handling, management of violence and aggression.
