Job summary
Applications are invited for the role of Discharge Facilitator within Blackpool Teaching Hospitals newly established 'Transfer of care Hub'. Consideration will be given to both full-time and part-time applications and the role is on a permanent basis. This new role will compliment the existing Discharge Services and will provide a service to our wards to enable safe and timely discharges for our patients. This is a very high profile role and as such is a key element of the Trusts future plans and strategy to reduce both internal and external delays.
Weekend working is a requirement (approximately 1 in 5 weekends currently).

Main duties of the job
To actively monitor the multidisciplinary management and discharge plan for all patients on designated ward(s) and take action to expedite the process, avoid delays and thereby improve the patients' experience.
A critical success factor in this role is reducing the length of stay and ensuring that the discharge is planned and executed for all patients. For some patients this will mean reducing stay by hours.
Act in a coordinating role between the Wards and Community health & Social Services (e.g. Neighbourhood Hubs)
Work in a way that is compliant with the Care Act (2014) and the Mental Capacity Act (2005)
Empower individuals by supporting them to make informed choices
Act as a central point for all enquiries relating to patient discharge planning within designated wards.
