This is a new and exciting role as the Integrated Therapy Team expands and develops following the successful pilot of preventing hospital admissions and the development of an in-patient Frailty pathway.
We are looking for a Physiotherapist/Occupational Therapist who has excellent communication and interpersonal skills and is passionate and committed to delivering excellent, evidence-based care. Experience of working within the field of Emergency Medicine and Clinical Frailty is essential, as the clinician is expected to provide advanced assessment and treatment to patients and a high level of clinical expertise.
The Clinical Specialist Physiotherapist/Occupational Therapist will provide clinical leadership to the therapy team and support the Team Leader with their responsibilities, as required. The successful candidate will work closely with the ED Consultants, Geriatricians, Nursing team and other members of the MDT. The role will also play a role in planning, co-ordinating, delivering and evaluating the service in conjunction with therapy managers.
Main duties of the job
To provide highly specialist therapy interventions to patients within the Emergency Department, Frailty Assessment Area, Short Stay Unit, Acute Medical unit and Same Day Emergency Care Unit.
To organise, monitor and evaluate the provision of a co-ordinated service to patients at Blackpool Victoria Hospital.
To manage a caseload of patients with highly complex needs, using evidence-based and client-centred principles to assess, plan, implement and evaluate interventions. Provide this service with a caseload of complex cases as an autonomous practitioner, demonstrating high-level problem solving and clinical reasoning skills, and independence in judgement.
To ensure patients receive continuity of care and seamless transitions between the acute hospital environment and the patients homes, in-patient rehabilitation settings, Residential care settings and onward referral to community services as required.
Close working and liaison with multi-professional groups, patients, relatives and carers to ensure maximum effectiveness of the service.To be professional, legally responsible and accountable for all aspects of the work undertaken and maintain up to date and accurate records as an autonomous practitioner.
