Job summary

An exciting opportunity has arisen to join Sandwell and West Birmingham Hospitals as a Band 3 Patient Flow Assistant within Emergency Care (Emergency Department and Acute Medicine).

The role will be based at Midland Metropolitan University Hosptals, Birmingham.

Shift Patterns are as follows:

  • Long Days 07:00 - 19:30 & Nights 19:00pm-07:30amMonday to Sunday
  • 34.5 Hours per Week - 3x Long Days or x3 Nights
  • 23 Hours per Week - 2x Long Days or 2x Nights

Ongoing development through apprenticeship standards can be supported through our staff personal development programmes once employed at SWB, there may be an opportunity to undertake a business administrator qualification once employed into this post.

Main duties of the job

The role of the Patient Flow Assistant (PFA) is to provide and ensure patients have planned timely transfers/ discharges and that the patient's pathway is effectively managed, ensuring all patients have a positive experience.

The PFA will be based across the Emergency Care Departments supporting and working under the direction of the senior nursing teams to help ensure that patients are pro-actively managed through the system. Checking that key elements of the patient pathways such as investigations, medications and transport are being requested and chased in a timely manner.

As a Patient Flow Assistant, you will also be expected to act as a single point of contact between the multiple disciplinary teams for the management of bed availability/ allocation across Emergency Care. The role will require consistent and coordinated approach to the operational management of Patient Flow, with the emphasis on maintaining an on-going approach of real time understanding of the Units current and predicted capacity position.

Another key element is to ensure relevant paper and electronic systems/ documentation relating to patient's medical records and plan for discharges and transfers are kept up to date and recorded on the appropriate systems and any delays are escalated appropriately.
