Nutrition and Hydration Therapy Support Worker

Job summary

To work as part of a multi-disciplinary team supporting the delivery of nutrition treatment programmes, using defined screening assessments, guidelines, protocols and competencies with guidance and supervision of Speech & Language Therapists, Dieticians and Nutrition Nurses. The post holder will visit patients who are residents of Care Homes and also those based within their own homes in the local community. The post will involve the opportunity to deliver individual and group teaching sessions. There is also opportunity for further development of knowledge and skills, and become a key member in a team who are striving for excellence in the care and treatment we provide to some of the most vulnerable people in our society.

Main duties of the job

To independently undertake and evaluate the outcome of screening assessments to identify patients at risk of malnutrition.

To implement and progress first-line, guided treatment programmes to support oral nutrition, following departmental protocols and progressing those programmes as appropriate without direct supervision and to provide feedback to the registered therapist regarding assessment of patient progress.

To monitor food/fluid and weight charts as part of a structured process for evaluating the progress of patients requiring nutrition support interventions.

To make structured observations of an individual's eating and drinking, including mealtime observations and monitoring of food and fluid consistencies as recommended by an SLT, including identification of signs and symptoms of dysphagia.
