Job summary
We are seeking an enthusiastic, driven senior nurse with excellent clinical skills to join the Department of Rheumatology. Our teams are based across our hospital sites .
We are looking for an individual that is highly motivated, able to work effectively within in a large multidisciplinary team and alsoto use their own initiative within their scope of practice.
The successful candidate will have responsibility for the management of Patients with complex conditions and will promote the service and deliver high quality care that is evidence based and reflects current best practice. They will work collaboratively within the Rheumatology team and be visible and accessible to patients, carers and staff. The post holder will provide highly specialised advice, education, training and support for patients and be responsible for the development of care pathways and nurse led initiatives to enhance services delivered.
Main duties of the job
For this role you must demonstrate that you are reliable and flexible. We care for patients with complex, multi-system health needs. We support patients from diagnosis and enable them to self-manage their long term conditions. We provide assessment and treatment, health education and support to patients to manage their conditions in clinic, via a telephone advice line and virtually using tele-medicine.
