This post offers an opportunity for a Dietitian to take the next step in their career and really make a difference to patients who have recently suffered from a stroke and are either undergoing rehabilitation, or living with and adapting new coping mechanisms following a stroke. We are looking for a community Dietitian who is excited by the opportunity to work and lead with the implementation of best nutritional care within this patient group.
Our New Integrated Community Stroke Service (ICSS) opened in early 2023 and the postholder will work with the community stroke services to provide nutritional support and dietary intervention to patients following a stroke diagnosis in a community setting. The successful postholder will work to support patients to achieve their potential, to maximise their recovery and to optimise quality of life in the short, medium and long-term.
Main duties of the job
You will hold a caseload of nutritional support and home enterally tube fed patients, working with patients to develop individually tailored plans to meet their nutritional needs. The caseload is rewarding and varied, working with people in their own homes, outpatient centres and care homes within an integrated multidisciplinary team across Sandwell & West Birmingham.
We are looking for an individual who is motivated by MDT working and who is driven to support the clinical delivery of dietetic care on the stroke pathway for our diverse population in a community Nutrition & Dietetics service. Clinically this post is integrated with the stroke team consisting of; Consultant in Stroke, Dietitians, Speech and Language Therapists, Nutrition Nurses, Physiotherapists and Occupational Therapists alongside multi-profession support staff.
Candidates will be joining our ambitious and forward-thinking therapies team in a trust that seeks to be the leaders of integrated care at a time of considerable opportunity for our service. You will need to use highly developed interpersonal skills to establish and foster strong relationships with a range of key stakeholders across the multidisciplinary team to support and enhance the clinical stroke pathway.

The Department trains student dietitians from Coventry and Birmingham City Universities across the full range of placements and this role will involve supporting the training of our students.
We are seeking a Dietitian to work with our community Nutrition and Dietetics team within community stroke services. The successful postholder will receive support from both the Integrated Community Stroke Service and the Nutrition and Dietetics team.
The role primarily focuses on the care and support of patients in their own homes, care homes and community clinics who have had a stroke diagnosis; it may also include the care and support for patients with neurological diagnoses.
The postholder will have the opportunity to attend and integrate with community MDT meetings and to be a key member of the community stroke team championing nutrition care on the community stroke pathway.
The role includes development, review and modification of nutrition-related resources and guidance.
