Job summary

We have a new role based in our Medicine Therapy team with Cardiology and Respiratory specialisation for a physiotherapist who is passionate about providing not only excellent Cadio-respiratory care, but who also thrives in progressing the rehabilitation and discharge planning of patients with complex co-morbidities

This post provides the opportunity to work within the integrated medicine therapy team ( both physiotherapists and occupational therapists), which provides therapy services to a predominantly frail elderly population of patients across both the Sandwell and City Hospital sites. Our Respiratory Hub which comprises of respiratory medicine and protected Non Invasive Ventilation (NIV) beds and the Cardiology wards are based at Birmingham City Hospital. A keen interest in assisting patients with the physical, psychological and social aspects to their recovery is important for this role. Our aim is for our patients to enjoy an improved quality of life by giving them the tools to understand and manage their cardiac and Respiratory condition.

This role will also give an opportunity for the succussful candidate to improve the existing therapy services within the cardio-respiratory wards through service development projects, implementing seamless pathways for patients from actue to community care and also be responsible for formulating thorough teaching and training for the medicine therapy team and the MDT. You will part of a 7 day rota including oncall service.

Main duties of the job

  • Supporting the ongoing rehabilitation of a patient who has moved from critical care to the respiratory hub, assessing for rehabilitation complexity and need, in order to facilitate appropriate transfer into community services for ongoing rehabilitation
  • The challenge of improving the physical and functional task abilities of someone suffering with a long term respiratory condition such as COPD
  • Educating other professionals on the prevention of hospital acquired functional decline through prolonged bed-rest via early restoration of physical activity
  • To work as an independent and autonomous practitioner within a multi professional team, ensuring that patients with multiple pathologies due to cardiac disease receive the highest standards of care.
  • Identifying patients who are end of life and signposting into the appropriate palliative care services
  • To provide professional support, clinical expertise and appropriate education and training to all those professionals involved in providing care to patients with cardiac and Respiratory disease.
  • Clinical supervision sessions with a junior member of staff whom you supervise

- To be the lead clinician for all cardio-respiratory inpatients, completing advanced therapeutic assessments and formulating individualised treatment plans.

-Provide expertise within acute medical inpatients including respiratory, cardiology, multiple pathology, neurology and older adults care management to physiotherapy and multi-disciplinary colleagues, patients and carers through specialist advice and education.

- To be responsible for defined areas especially Cardio-respiratory wards and own caseload without supervision. To have advanced knwoledge of management of patients with various cardiac conditions and linking in with cardiac nurses in their rehabilitation.

- To be responsible for service development within the cardiology and Respiratory wards linking in with key members of the MDT.

- To liaise with community therapy team to implement seamleass pathways for continuity of care for cardio-respiratory patient from acute to community setting- cardiac and pulmonary rehabs services.

- To formulate and conduct thorough teaching and provide relevant training to the therapy teams especially Medicine in management of cardio-respiratory patients.

- To work closely with the respiratory therapy team and Critical care nurses for continuity of care of patient stepped down from ITU/CCU and patient needing Non-Invasive Ventilation

-Provide clinical leadership to Band 6 & 5 Physiotherapists, occupational therapist and therapy support workers within the speciality through supervision, assessment, education and development

- Lead the supervision and assessment of undergraduate/post-graduate Physiotherapy students within own clinical area as required ensuring the standard of practice and teaching meets the standards set by a degree/higher degree level.

- Will be part of the oncall service and 7 days working rota and potential extended hours in line with the respiratory hub service.
