Community Mental Health Practitioner

Job summary

This is an exciting opportunity for an experienced clinician to come and join our Mental Health Together + team based at Gregory House, St Martins Hospital in Canterbury.

The role will involve working within our Community Mental Health team, supporting all age adults with enduring mental health concerns. The RMN/Occupational Therapist is an experienced practitioner within the multi-disciplinary team.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

Work effectively as a clinician in the Community Mental Health Teams for all ages and Older People, ensuring that patients referred for assessment experience a smooth pathway through the referral, diagnosis and treatment process.

Be effective within the duty team, answering phone calls and directing patients/carers/services to the appropriate outcomes required.

Be proactive in responding to urgent situations within the MDT.

Be able to undertake a clinical risk assessment and act accordingly to the presented risks.

Be able to manage highly stressful situations in an organised manner.

Contribute to the planning and implementation of interventions.

Monitor clients' progress, reporting on clients' psychological, physical, behavioural and social needs.

Adapt and evaluate interventions, ensuring that they support the client and his/her carer to meet their needs, adjusting own approach accordingly.

Be effective at care planning and liaising with other agencies to provide holistic and person centred care.

Be effective at time management and manage a case load.
