Job summary
This is an exciting opportunity for a Healthcare Worker to join our experienced multi-disciplinary team at the Rosebud Centre (Inpatient Rehabilitation Unit).
Rosebud Centre is comprised of 10 single bedrooms offering 24-hour care 7 days a week for individuals with complex mental health needs. It is situated in Leybourne, West Malling and is one of six Rehabilitation Units across Kent, which provides inpatient Recovery and Rehabilitation.
Under the guidance and supervision of the team, you will deliver an effective and competent level of care to adults with mental health rehabilitation needs, in the Unit and the local community. You will be involved in assisting patients in their day-to-day activities, supporting them to maintain and develop skills towards independence, and continue their recovery.
This post will involve shift working between to cover 24 hours, 7 days a week.
Main duties of the job
The post is suitable for non-registered support staff, with prior experience working with adults with mental health difficulties.
You will be supporting registered staff in carrying out a variety of mental health needs assessments & interventions, both with individuals, their carers & groups.
You will be promoting patient's self-care and supporting with activities of daily living with reference to the care plan.
You will need to be confident & competent in working with individuals in an inpatient rehabilitation setting based in the community & be able to work effectively in a multi-professional context.
You should be a good communicator, & are able to adopt a flexible, adaptable & proactive approach to your work.
You willwork with a defined caseload of clients who have identified rehabilitation needs as a result of mental health issues using specific approaches as directed by the clinicians responsible for the care plan in a recovery focused & socially inclusive manner.
This post consists of working days & occasionally nights as per rota roster.

* Effectively establish a therapeutic relationship with service users and their relatives/carers.
* Support individual risk assessment and management plans in line with service and Trust policy.
* Assist in the assessment of clients with complex mental health needs in order for the clinical team to form an accurate diagnosis of impairments, strengths and capabilities and formulate clinical needs.
* Involve individuals in the planning and prioritisation of their activity plans wherever possible.
* Work collaboratively with service users, staff, and relatives to deliver an appropriate intervention/rehabilitation/recovery plan which aims to maximise engagement and function whilst balancing risk, and promoting choice and independence.
* Support clients and the clinical team to identify relapse indicators and alert relevant staff to any changes in the client's mental state.
* Monitor rehabilitation programmes and feed back accurately to the team in order to measure progress and increase efficacy for service user outcomes.
* Support the clinical team to identify client's needs for additional care provision / housing/specialist placement/further assessment /referral on to other agencies.
* Support the assessment of service users referred to the rehabilitation service where appropriate.
* To plan and manage own delegated workload on a day to day basis.
