Job summary
We have an exciting new opportunity within South Kent Coast CMHTs as we look to develop a brand new Senior Mental Health Nurse position providing guidance and support to the team at a senior nurse level.You will have continuous responsibility as lead Health Care Professional for a defined caseload of no more than 40 patients. You will liaise between stakeholders to ensure clear communication and seamless care pathways, thus enabling access to the appropriate level of care and intervention.You will provide senior clinical leadership within the locality and work collaboratively with our Operational Managers and Specialist Community Nurses to provide advice on all nursing issues with an expectation to provide specialist mental health advice and education to colleagues.
Main duties of the job
Risk assessment and risk management is an essential aspect of this role and in addition to the expectation to participate in providing quality first line psychosocial assessments, the role also requires working proactively in forging professional working relationships with multi-agencies.You will facilitate clinical sessions offering short-term interventions based on psychological therapies and offer professional advice to patients and their carer's, other professionals, statutory and voluntary agencies. You will facilitate the transfer of patients between both primary and secondary care and between specialist teams within the Trust.You will either work as lead Health Care Professional to those on both standard and enhanced CPA.The post holder will work with the Clinical Nurse Specialists on the setting, monitoring and maintaining standards of care and will advise the Operational Managers and Locality Manager on issues which endanger standards of practice within their specific area of practice.You will assist the service in facilitating and developing care pathways within CMHTs.If this sounds like your next role to further expand your clinical expertise then we would like to hear from you.
