Job summary

Main duties of the job

You will be key within our multidisciplinary team. You will lead holistic and autonomous nursing care whilst maintaining robust communication with other professionals and agencies. You will have responsibility to assess, plan, deliver and evaluate care and communicate your findings to provide the best possible care to residents of South Kent Coast. As we move away from CPA we deliver interventions aligned to KMPT clinical care pathways thus providing structure, purpose and clarity for people accessing our services.

Effective communication skills and self-governance is essential. You need to demonstrate knowledge of secondary care interventions and demonstrate assessment skills. We are keen to work with people to identify strengths and interests therefore roles can be a combination of different functions or indeed a specific function; These functions include;

Duty - Urgent Care

Interventions - upskilling and clinical supervision provided

Depot and Clozapine clinics with a focus on Serious Mental Illness and physical wellbeing.

Active Support

You will be required to provide clinical leadership in the absence of more senior colleagues, lead on core functions and provide operational supervision to a designated number of care support staff.

You will be expected to provide interventions to both the Dover/Deal & Shepway localities.

  • Registered on NMC
  • One year experience at Band 5 or above in hospital or community health care
  • Evidence of continued professional development, appropriate to role
  • Clear understanding of CPA and care coordination role

Desirable

  • Post-registration certificate ENB 998. Possession of specialist nursing or other relevant highter academic or skills focussed awards
  • Post registration mentorship qualification
