Community Mental Health Nurse/Occupational Therapist

Job summary

The post is suitable for registered nurses, occupational therapists, health professionals with experience in physical health or learning disability, with at least 2 years post qualification experience. You will need to have demonstrated skills and expertise in carrying out a variety of assessments and be able to plan and execute interventions, both with individuals, their carers and groups. You will need to be clinically confident and competent in working with individuals in community setting and be able to work effectively in a multi-professional context. You should have collaborative and enthusiastic styles as well as have a passion and commitment to develop and promote service provision for older adults with mental health and dementia related needs.

Main duties of the job

The assessment , treatment and support of individuals experiencing mental health disturbance, within a community setting

Working within an MDT to collaboratively provide an optimal level of care to individuals referred to the service.

To be responsible for maintaining professional registration. To adhere to KMPTs policies, procedures and guidelines. To ensure that the Trusts values are upheld in practice. To be professionally and legally responsible and accountable for all aspects of ownwork. To provide effective supervision, support, and guidance for identified other staff andany work delegated to them. To take into account the individuality of the views, wishes, values and beliefs of serviceusers, and relatives, respecting equality and diversity and contribute to the provisionof a service sensitive to these needs. To effectively liaise with service users, family, carers, advocates and health and socialcare professionals regarding service users assessment, treatment and care to ensurecontinuity of care and delivery of a coordinated service as a member of the MDT To ensure all communication adheres to Trust policy and Caldicott / data protectionprinciples. To assess capacity and obtain informed consent from service users and work within alegal framework with those service users who lack capacity to consent to treatment. To ensure service users are involved in the planning and prioritisation of their careplans wherever possible. In conjunction with colleagues to undertake, record, review and work within individualrisk assessment and management plans in line with service and Trust policy. In conjunction with others to identify service users needs for additional care provision/ housing/specialist placement/further assessment /referral on to other agencies asrequired. To critically evaluate national guidelines, audit results, research, evidence basedpractice and legislation to develop and improve service provision. To be responsible for the monitoring equipment and materials, ensuring sufficientresources to carry out the job, and including the responsible use of petty cash. To be responsible for the development of identified others, utilizing and providing thesupervision and appraisal process in order to ensure contribution to the Trusts, andthe Service objectives. To undertake statutory and essential training in accordance with Trust policies andprocedures. To demonstrate on-going personal development through participation in internal andexternal development opportunities, including audit and research activity, recordinglearning outcomes through maintaining a CPD portfolio. To fulfil the role of mentor/practice educator, maintaining own update training andensuring responsibility for regular placement provision. Ensuring supervision andwritten assessment of students whilst on placement. To attend, and actively participate/lead relevant clinical meetings/forums. To act as a preceptor for newly registered staff or those returning to practice.
