An exciting new opportunity has arisen for a dynamic, motivated individuals to join the Review and Resettlement Team.
The post holder will work as a member of the Out of Area team and be actively involved in the discharge planning process of all patients to improve the patient journey, minimise patient delays. The post holder will work under supervision of the Complex Care Coordinator to collaborate with other professionals in contributing towards discharge planning. The post holder will be responsible in supporting the Complex care Coordinator with complex discharges and supporting the patients to settle in their new environment for a minimum period of 3 months and facilitate transition to back CMHT
This role allows you the flexible to maintain good work-life balance includes options of working at any of our KMPT locations across Kent and home.
You will have good experience of supporting patients with complex mental health and needs. You work will closely with CMHT and Social services to support Patients settle back in the community when repatriated back to Kent.
You have good knowledge local services that will support the patients with integrating back in the community.
Ability to travel across a wide geographical area in a timely and flexible manner in accordance with the needs of the job. Preferably a driver who possession of clean driving licence.
Main duties of the job
- To work as a core member of the Review and Resettlement team in their across the RRT case load and contribute to discussions around discharge planning.
- To work collaboration with Complex care coordinator and social care professional to identify the need of the patients on discharge.
- To follow Trust policies and procedures in relation to Risk Assessment and Care Planning
- To work flexibly in order to respond to crisis work, and to prioritise those patients who require support.
- To provide practical support to patients including help with integrating back in the community.
- To help patients gain access to a range of resources and services such as benefits and welfare rights.
- To support patients with home visits and/or visits to identified placements.
- To support patients to integrate back into the local community and work closely work with CMHT to facilitate transition back to CMHT .
- To communicate with members of local mental health services, other mental health professionals and other agencies to allow consistent and comprehensive care to be provided to patients.
- To receive clinical supervision and appraisal from allocated Discharge Coordinator To complete all mandatory training and undertake further training in order to support role development.
- To ensure record keeping is accurate and all Caldicott principles and procedures are undertaken.
- To complete any other tasked allocated by supervisor

- Ensure services are able to be delivered safely and in accordance with the requirements of the Health and Social Care Act, including monitoring the quality and safety at each site of responsibility, identifying breaches, escalating them and acting on them as soon as possible.
