Job summary
An exciting new opportunity has arisen for a number of qualified Advanced Clinical Practitioners (ACP) within our Older Adult Community Mental Health Service located in various sites across Kent. These highly skilled roles are designed to transform and modernise pathways of care, as part of an ongoing system-wide mental health transformation programme in line with the national Community Mental Health Framework (CMHF).
Main duties of the job
The ACPs, will be Independent Prescribers (IP), who will provide expert clinical input to older people with functional mental health problems, and be able to deliver specialists interventions. Their specialist older adult knowledge and expertise will be invaluable in supporting the teams with the older adult functional mental health pathway, in order to provide enhanced capacity, capability, productivity and efficiency. The post holders will have an advanced level of autonomy and will be supported to work in accordance with the four ACP pillars of clinical practice, leadership and management, education and research.
The scope of clinical practice will include carrying out clinical assessments, formulation, independent prescribing and holistic treatments. The post holders will need to demonstrate strong leadership and will work in partnership with patients, their carer's and internal/external colleagues within the MDT to deliver high quality, evidence-based care.
The posts will be hosted by the Community Mental Health Services for Older People (CMHSOPs), of which each team consists of Team Leaders, Lead Nurse/IP, Consultant Psychiatrists, Psychologists, Community Psychiatric Nurses (CPNs), Occupational Therapists (OTs) and Support Workers.
