Job summary

You will be joining KMPT at an exciting time. The work around the implementation of the Community Health Framework for Mental Health will enable Social Work to develop a strong profile within secondary care mental health service to improve patient experience and compliment our existing MDT.

As a social worker you will provide a high standard of support and care delivery with regards to social care needs to those service users whose ability to cope with the tasks of living are threatened or impaired by mental health issues, emotional, psychological and social problems.

You will be required to support secondary care mental health services throughout West Kent offering the successful candidate a choice of base between either Maidstone & Tunbridge Wells.

Main duties of the job

You will work closely with other agencies and individuals to support service users with a clear focus on recovery, social inclusion and social justice. The role will entail assessment, support planning and the opportunity to provide 1-1 strength based therapeutic interventions.

You will act as Lead Health Care Professional, (where applicable) with individuals who have serious and enduring mental health difficulties.

Develop and deliver evidenced based psychosocial interventions where appropriate. This is to include the Change Model for individuals diagnosed with Complex Psychological needs (Personality Disorders) and undertake training where appropriate.

Undertake holistic social work assessments using strengths based model collaboratively with services users, relatives and carers in order to promote the appropriate support towards recovery.

Undertake a range of procedures and practices underpinned by theoretical knowledge or relevant practice to provide a service that is tailored to meet the needs of the individual.

Ensure effective communication with particular regard for individuals who may have difficulties in relating to the complexity of emotional and psychological conditions.

This will include skills in managing moods, motivation, counselling, training,empathising and reassurance.

Manage an appropriate caseload requiring social work interventions.

Ensure appropriate risk assessments and risk management plans are completed and reviewed.

Ensure all child protection, adult safeguarding, Mental Capacity Act issues are dealt with according to policy and procedure.

Undertake the function of Care-Coordinator in line with Trust policy for the Care Programme Approach where appropriate.

Work collaboratively and in partnership with service users, relatives, carers and others in order to seek community based solutions.
