We have one vacancy left for a band 6 Specialist Practitioner, the post is
open to RNLD Nurses or Occupational Therapists with extensive Learning
Disabilities experience.

people with a learning disability in the community who may be experiencing a
mental health crisis or have complex needs, in order to
avoid placement breakdown, inappropriate hospital admission, and risk of
contact with the criminal justice service.
You will need to be a
confident decisive individual who can
work autonomously with a positive attitude and
a commitment to provide the best standard of care and promote a high quality of
life for people with learning disabilities across Barnsley, Calderdale, Kirklees
& Wakefield districts.
Experience of multi-disciplinary working, positive behavioral support
model and delivering a crisis response in the community would be
highly valued.
The hours of the service are Mon-Sun
20:00-08:00 and Sat/Sun 08:00-20:00.

Main duties of the job
Promote a positive behavioural approach, underpinned
by professional skills and additional training as needed
Working
alongside other professionals and agencies both within the Trust and across the
wider care sector providing specialist advise and support using appropriate
assessment tools based on evidence based best practice and promoting the Green
Light process.
Manage
and respond to crises as per protocols of the Trust.
Maintain
up to date records and activity data as per professional and Trust standards
Demonstrate
a broad level of understanding of the effect of disability, provide advice in
the MDT assessment of service users and carers, work proactively with ward
staff, community agencies and voluntary agencies to provide a seamless service
for service users.
Maintain
effective communication within the team, liaise with other agencies, voluntary
and statutory sectors ensuring service user needs are met.
Empower
families and carers to support individuals with learning disabilities and
involve service users in all aspects of their care.

Job responsibilities
The post holder will be part of a
cohesive and enthusiastic multi-disciplinary team and work closely with the
other parts of the locality health team and In-Patient team where required. You will be involved in providing specialist assessment,
support and treatment for individuals receiving intensive support and care,
enabling service users to engage in a wide range of individual and group based treatment
activities.
The

KEY RESULT AREAS:
Clinical
1.
To manage an individual caseload, providing
assessments for service users receiving
in-patient care and within the community, prioritising clinical
needs and providing an effective service.

To work with service users to identify goals as
part of the overall care plan, using appropriate assessment tools based on current
evidence based best practice.

To plan and implement a client centred
treatment programme on an individual and/or group basis, to achieve therapeutic
goals.

To monitor, evaluate and modify treatment in
order to measure progress and ensure effectiveness of intervention.
