Psychiatric Liaison Practitioner

Job summary
To provide a psychiatric liaison service into Mid Yorkshire Hospitals Trust sites which will provide assessment, support, advice and training. To provide highly developed and skilled assessment, screening and interventions for a wide variety of Service Users in Acute Trust settings and to signpost and broker Service Users toward relevant and appropriate resources to meet their needs. To be a part of the 24 hour staff rota in order to ensure the service is accessible at all times, referrals are received and screened promptly and face to face assessment are undertaken within 1 hour of referral where urgent. To participate in the provision of staff supervision and professional development.
The service is ageless, the team will provide a 24hr liaison assessment and support within Emergency Departments and the inpatient units across Mid Yorkshire hospitals sites
This service will cover 24hrs working on 12hr 2 shift system covering days and nights. All applicants must be able to undertake all shifts covering day and night duties. Some flexibility will be essential to ensure service delivery is maintained.
Successful applicants will be expected to display a range of adaptive skills and leadership qualities, with excellent communication and assessment skills, a good working knowledge of Primary, Secondary and third sector providers across the locality is essential.
Main duties of the job
- To assess patients in a timely manner across the acute care pathway and wider acute trust who present with a range of mental health issues.
- Following assessment, signpost in to appropriate services
- Complete comprehensive risk assessments to support discharge and ongoing referrals
- To support patients whom attend with multiple co-morbidities
- The candidate is expected to work across three sites
- To work autonomously in the role
- To work a shift pattern over a 24 hour period
- To participate in staff supervision
