15 hours per week.

An exciting opportunity has arisen for a band 5 community mental health nurse, join our multidisciplinary team to prevent hospital admissions nursing service users in their own homes where their needs can be met through the delivery of high-quality care having an holistic approach to the well-being of individuals in the community and striving to make a meaningful difference in their lives. The ideal candidate will be passionate, highly motivated and be committed to tackling health inequality along with having excellent people, planning and organisational skills.

Main duties of the job

The successful applicant will be part of a multi-disciplinary team delivering comprehensive assessments and a range of approaches, interventions and treatments and treatments to people experiencing mental health issues with the aim of supporting them with their personal recovery.

The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.

Phlebotomy trained is preferred or prepared to train is essential due to the physical health interventions required within the role.

You will be required to hold professional responsibility for a number of service users, and take the lead role in their care. This reflects a need to demonstrate excellent communication skills and be able to work collaboratively within a multi-disciplinary team. You will be committed to the delivery of a high-quality service, working both independently and as part of a well-established, innovative and recovery focused team in meeting the needs of service users and their carers.

In addition to holding a caseload, the successful applicant will be offering initial assessments to clients referred to the service and be supporting individuals in mental health crisis in the community.

Workload Management

  • To work flexibly to cover shifts to provide a 365 day per year service
  • Act as shift co-ordinator as required and to participate in the planning and co-ordinating of the shift to ensure smooth and effective running of the service and role model best practice
