Job summary
Full Time = 37.5 hours per week.

We are the Learning Disabilities Out Of Hours service and are looking at expanding our team in order to cover the out of hours times of Monday to Sunday 20:00-08:00 and Saturday and Sunday 08:00-20:00 for which the agenda for change enhanced pay rate is applicable. The posts are open to experienced RNLD nurses and Occupational Therapists with extensive Learning Disabilities experience.
The aim of the service is to provide timely interventions, support and advise in order to avoid placement breakdown, inappropriate hospital admission and prevent the risk of contact with the criminal justice system, to people with a learning disability, their family and carers and other professionals.
Applicants will need to demonstrate positivity and commitment to provide the best standard of care and promote a high quality of life for people with learning disabilities across Barnsley, Calderdale, Kirklees & Wakefield districts. We seek confident, values-led clinicians who can demonstrate autonomous clinical decision making for service users presenting with a range of complex needs.
Main duties of the job
Promote a positive behavioural approach, underpinned by professional skills and additional training as needed
Manage a caseload providing assessments for service users, prioritising clinical needs, and providing an effective service, using appropriate assessment tools based on evidence based best practice.
Manage and respond to crises as per protocols of the Trust.
Maintain up to date records and activity data as per professional and Trust standards
Demonstrate a broad level of understanding of the effect of disability, provide advice in the MDT assessment of service users and carers, work proactively with ward staff, community agencies and voluntary agencies to provide a seamless service for service users.
Maintain effective communication within the team, liaise with other agencies, voluntary and statutory sectors ensuring service user needs are met.
Empower families and carers to support individuals with learning disabilities and involve service users in all aspects of their care.
