Full Time - 37.5 Hours Per Week
This new post provides an exciting opportunity for a qualified and
experienced mental health nurse, social worker or occupational therapist to
work alongside community mental health teams in Wakefield to enable and ensure
smooth transitions from Children's to Adult Mental Health Services for Young
People and their families.
The role of
Young Persons Transition Lead MH Practitioner will involve clinical leadership,
line management and supervision responsibilities as well as assessing mental
health needs and delivering direct interventions with Young People.

Main duties of the job
As the Young Persons Transition Lead MH Practitioner you
will ensure best practice and provide clinical leadership and expertise.
You
will work alongside CMHTs, CAMHS and Wakefield Council to ensure smooth
transition for young people moving from Children's to Adult services.
You will
be a member of the leadership team and be responsible for the delivery of safe
and effective clinical practise across the community mental health pathway.
You
will work alongside the Adult MH teams that are multi-disciplinary and
integrated with Wakefield Council.
You will need to develop excellent working
relationships across Adults' and Childrens' services, acting as a role model
for exemplary practice.
We are looking for candidates who are highly motivated,
committed, confident, with energy, drive and enthusiasm and the ability to work
under pressure.
You must have substantial post qualification experience, some
of which will have been in a mental health or other complex care setting and
have experience of working with Young People.
We are looking for candidates
with experience of working in a multidisciplinary team who can provide
leadership to all staff in a way which encourages the development of a high
quality, responsive and evidence based service for Young People with mental
health problems.

individual presentations and complex situations, synthesising information from
multiple sources to make appropriate, evidence-based judgements and/or

which may include prescribing medicines, therapies, life style advice and care.
- Exercise professional judgement to manage
risk appropriately, especially where there may be complex and unpredictable
events and supporting teams to do likewise to ensure safety of individuals,

- Work collaboratively with an appropriate
range of multi-agency and inter-professional resources, developing, maintaining
and evaluating links to manage risk and issues across organisations and
