1 x 30 hour post to cover maternity leave until March 2025
We are looking for an
enthusiastic and motivated Clinical Pharmacist to be part of the Calderdale and
Kirklees Locality mental health pharmacy team, working up to 0.8wte to cover the maternity leave of the
existing post holder.
As a registered pharmacist
with some post registration experience, you will be supporting the pharmacists
in the Calderdale and Kirklees locality of our trust with the day-to-day
pharmaceutical management of service users within our care.
A background
in hospital pharmacy practice, a clinical post graduate diploma (or equivalent
experience) and experience in psychiatry are desirable but not essential as
there will be opportunities for further clinical training. The
post is also part of the wider Trust pharmacy team with access to support, training,
and supervision from experienced mental health pharmacy staff.
The
successful candidate will have excellent communication skills and be able to
understand and interpret clinical information and communicate it to patients, carers,
and other health professionals.

Main duties of the job
The post holder will provide
integrated clinical pharmacy services to inpatient wards and community-based
teams and be a member of the multidisciplinary team (MDT). The post will be
based in the inpatient unit at the Dales, providing cover for inpatient units /
intensive home-based treatment teams / rehabilitation units, with opportunities
for involvement in; MDT audits, support to community teams, point of care
clozapine service, development / delivery of training to the wider MDT.
The post holder will also
support the departmental and trust wide governance agenda, providing clinical
input into audit and service development projects. They will support the
day-to-day operation of the service, providing training and staff supervision
as necessary.

The post will be mostly based at The Dales Unit, Halifax but may be
required to travel to other localities within Calderdale and Kirklees to

Folly Hall Mills in Huddersfield.
The post holder will be
required to participate in the Saturday morning and out-of-hours on-call rota.

- To undertake therapeutic drug monitoring for relevant medications in order to optimise efficacy and tolerability (examples include clozapine, lithium, anticonvulsants etc)
- To undertake clinical audit and practice research within the Pharmacy Department where appropriate.
- To record clinical interventions and complete query record forms in line with departmental policy. To collate all Trust pharmacist clinical interventions and report on trends and outcomes where requested as appropriate by the Principal Pharmacist / Senior Clinical Pharmacist.
- Promote evidence based cost-effective prescribing.
- To provide professional and legal clinical supervision in the dispensary and to participate in the checking and dispensing of prescriptions (including compliance aids) when required as directed by the Technical Services Manager.
- To develop and maintain links with other professionals, such as dieticians, nursing and medical colleagues regarding service developments and improvements. To recommend service changes and implement them where appropriate. This may include changing paperwork, changing storage or delivery arrangements etc.
- To support nursing staff and Medicines Management pharmacy technician by providing clinical pharmacy advice to the patients self-administration scheme and to undertake ongoing evaluations of individual outcomes of patient self-administration.
- To participate in the appropriate reporting process: to report, record and monitor medicines related incidents or complaints and to explore ways of minimising risk of future re-occurrence or risk to staff.
- To undertake the dispensing of clinical trials in accordance with the dispensing protocol, ICH GCP guidelines and European Directives.
