Job summary
The FCTT/SCFT
focus on supporting service users with complex mental health needs within a
community setting. You will support them to develop the skills needed to engage
in a range of individual and group community-based treatment activities. It
includes service users with a range of complex presentations, the role will
involve working with statutory and third sector providers to develop and
deliver innovative packages of support and care in the community as an
alternative to in-patient care or to support transition from hospital to a
community setting.
Both the FCTT and
SCFT have transition pathways out of hospital into the community across the
West Yorkshire. The postholder will help to embed and engage our service users
in recovery-based opportunities, and interventions, that create positive
attachments to their onward setting. The vision is to improve quality of life,
self-esteem, working relationships, personal identity and to reduce length of
stay in hospital settings.
The successful
applicant will work within our highly-motivated multi-disciplinary team to
provide therapeutic interventions with an emphasis on recovery. As the post is
shared across FCTT and the SCFT working pattern will be 2 mid shifts and 2 long
days providing cover from 8am - 8pm . The community work will also require
agile working and equipment will be provided to support this. Being a car
driver is essential.
Main duties of the job
The
All-Age Forensic Community Service is a developing service line within the
Forensic Care Group. It includes:
Forensic
CAMHS (inclusive of Y&H FCAMHS, Focus Secure and Focus Wakefield)
Forensic
Outreach and Liaison Service for Learning Disability and/or Autism (FOLS LDA)
Our adult
forensic community service (Forensic Community Transition Team (Bretton Centre)
and the Specialist Community Forensic Team (SCFT))
The
vision is to create a collaborative environment between our community forensic
services and to develop and overarching clinical framework that will support
the way in which a community forensic population is understood, supported and
intervened upon, no matter what age. The purpose of the role is therefore to:

in the case management of service users.

assessments and risk assessments

individuals.

Support
service users with personal care duties (bathing, toileting, dressing, support
with meals, assist patients with their appearance), as part of a
healthcare team, provide the highest standards of safe and compassionate
planned care to people that maximises their recovery and rehabilitation
potential.
Undertakes
a limited range of delegated clinical care duties relevant to the service
provided/work area and records patient observations: e.g. taking blood
pressure, blood glucose monitoring, pregnancy testing, observing patients for
signs of agitation or distress, removal of peripheral cannula urinalysis,
removal of catheter.
participate in a flexible
rostering arrangement providing 24-hour/7 day care (as appropriate to the role).
