We have an exciting opportunity for an enthusiastic Band 4 speech and language therapy associate practitioner to join the Forensic Outreach Liaison Service for Learning Disability and/or Autism (FOLS LDA). The FOLS LDA service work with adults with a diagnosed learning disability and/or autism who are at risk of contact, or in contact with, the criminal justice system. The service covers West Yorkshire and Barnsley and is a multi-disciplinary team of professionals. FOLS is an evolving, dynamic service that provides a formulation driven, trauma informed and strengths-based approach to risk management. Clinical decisions and risk management are regularly reviewed together as an MDT. The team provides risk assessment, direct therapeutic interventions and supports transitioning out of secure settings to patients, as well as consultation, advice, supervision and training to relevant services and partners.

This post is suitable for someone experienced in supporting people with a learning disability and/or autism. They must be comfortable working with patients who present with risk behaviours and a strong communicator. As a community post, there is no direct personal care.

Main duties of the job

This is an exciting opportunity for a B4 associate SALT practitioner, ideally with some experience working with learning disabilities, Autism and mental health. You would work as part of an MDT alongside partner agencies to keep people safe, promoting communication and independence.

The post holder will contribute to a specialist communication service, using skills to assess, advise and provide appropriate group and individual interventions to ensure individuals communication strengths and challenges and recognised, supported and maximised, under the guidance of a qualified SALT. The team take an MDT approach to risk management and review cases regularly together. The successful candidate will receive training to bolster their skills.

The work will be undertaken in a variety of settings within community, secure and inpatient settings. The post holder will liaise effectively with all relevant professionals, carers, and family members to ensure the communication needs of each individual are recognized and supported.

Job responsibilities

  • Undertake delegated duties under the supervision of registered health care professionals. Consulting as necessary and working collaboratively within the health care team to deliver therapeutic interventions, psychological and physical care to a defined group of service users.
  • Deliver evidence based planned care, which is safe and effective, in a manner which is caring , compassionate and enables service users to reach an optimum level of functioning, independence, rehabilitation and recovery.
  • Provide care within well established policies, procedures, protocols, guidelines, and established clinical care plans, whilst acting within the required sphere of clinical competences for the role at all times.
  • Following initial assessment by a registered health care professional, exercise initiative and judgement in, care planning, goal setting, delivery, implementation and evaluation of care.
  • Working collaboratively and in partnership with others, develop knowledge to promote holistic care.
  • Demonstrate sound knowledge of the care and treatment programmes of individual service users to enable the post holder to contribute and inform the decision making of registered health care professionals.
  • Make appropriate care adjustments, within the agreed level of autonomy for the post, and be able to communicate any changes to service users, carers and the health care team. This may include regular assessment, evaluation, reporting and escalating relevant information to appropriate health and social care professionals.
  • Undertake enhanced training and preparation to meet the specific clinical needs of the service and to equip the post holder for an enhanced leadership role.
