Job summary
The specialist rehab and
recovery service in Calderdale have an exciting opportunity for a 12 month secondment
for a Band 6 Senior Occupational Therapist. This is an excellent development
opportunity. The role covers both the Community Based Rehab Team and the
Community Rehabilitation Unit. The role is supported by a Band 6 SOT, Band 5 OT,
and 2 Band 3 OTAs.
The core function of the
service is to provide rehabilitation interventions to support people to return
to a level of function that they were prior to the onset of their illness. The
Service also looks for opportunities for integration into the community to
engage in meaningful activities and occupation around areas of interest. The
service looks at what accommodation might be appropriate to the individuals
needs, if there is any deficits identified. All individuals working in the team
will be committed to providing a holistic therapeutic approach, opportunities
and relationships that deliver a positive experiences. This will be achieved by
co production of care plans to achieve the maximum recovery status for that
individual and promoting self -management of symptoms of wellness. Lyndhurst
Hospital is a 14 bedded level 1 Community Rehabilitation Unit and the
community-based rehab team offers up to 36 hours a day of rehabilitation in the
community.
Main duties of the job
The key duties and
responsibilities within the Band 6 Occupational Therapy role include,
Completing
assessments to establish baselines and understanding of Occupational needs, Supporting
engagement in meaningful activities. Completing individualized and group risk
assessment to manage risk and promote positive risk taking. Providing
collaborative tailored interventions based on mental health, sensory, and
specific communication needs, that promote access to occupation, based on clear
clinical reasoning and evidence-based practice. Including Practical skill
development, Therapeutic skills programs, and restorative interventions. Providing
regular repeat assessments and discussions in key development areas to
demonstrate recovery. Regular involvement in wider internal meetings including
management meeting, referrals, patient flow, and team meetings. Developing a
range of group or individual Well-being workshops to promote development of
life and self-management skills. Completion of monthly lead OT audit report to
reflect service achievements, engagement, and access to occupational therapy
services. Completion of 3-monthly About OT surveys, and completing structured
comparative reports to identify trends, development areas, create clear action
plans, and implement service development. Working in collaboration with the
wider Occupational Therapy Team to develop the service across sites.

JOB SUMMARY

in-patient and community rehabilitation services, enabling service users to

Clinical

interpret the assessment results in order to plan and implement a client
centred occupational therapy treatment programme on an individual and/or group
basis using graded activity to achieve therapeutic goals.

To
ensure therapeutic interventions are of a high professional standard, informed
by evidence based practice and involves the service user and carer in all
aspects of the care programme.
1.4. To
demonstrate knowledge and understanding of the effect of disability providing
training and advice on lifestyle changes/adaptations to the clients social and
physical environments.
1.5. To
act as the care programme co-ordinator as required and to offer carers an
assessment and care plan where appropriate.
1.6. To
contribute to a co-ordinated team approach throughout the assessment and in
meeting the service users needs including developing and maintaining close
working relationships with all relevant services, and agencies within the
voluntary sector.
1.7. To
regularly evaluate the effectiveness of group and individual occupational
therapy interventions by undertaking specific audits and using outcome measures
such as standardised / other relevant assessments and service user satisfaction
questionnaires.

continue treatment interventions where necessary to assist in the transition of
skills from an inpatient setting to living independently within the community.
1.9. To
work effectively within the multi-agency team ensuring that both written and
verbal communication is clearly maintained to ensure a seamless service.
1.10. To
be responsible for the maintenance of appropriate clinical records concerned
with service user treatment, in accordance with risk management documentation
guidelines.
1.11. To
arrange/attend appropriate clinical team meetings, referral meetings, case
conferences, care programme approach meetings and any other as required by
clinical need and as direct by the Line/Service Manager.

provide accurate and timely reports, statistical data through the electronic
recording system and other information as required.

participate in monthly clinical and managerial supervision and to provide
