Health Care Support Worker

22.5 hours per week - Internal Only

Calderdale Core pathway is a large pathway which comprises of Nurses, Social Workers Support workers, psychologists, and Medical Staff. As team members we operate to new ways of working. The Core pathway works with different types of service users. People within core pathway will be offered short term brief focussed interventions before they are discharged back to primary care. These brief interventions comprise of anxiety management / graded exposure work / accessing community facilities and third sector services.

As part of the role of the health care support worker core pathway you would be expected to :

Assess service user needs under the supervision of a senior staff member.

Hold a caseload of service users to who they will be providing an intervention.

Provide support to service users that is tailored to their needs.

Complete outreach visits to service users who are unable to come independently into clinic and/or facilitate supporting service users into clinic.

Work with service users to formulate a collaborative plan of care for their needs.

Help service users stay mentally and physically well and work towards an optimum level of recovery including liaising with GPs and other health care providers.

To work closely with GPs, physical healthcare providers, partner organisations.

A key role in this pathway will be to work closely as part of a team and create positive relationships with GPs and other agencies

Main duties of the job

Hold a small caseload of service users under the supervision of a qualified nurse.

Provide support to service users that is tailored to their needs.

Complete outreach visits to service users who are unable to come independently into clinic and/or facilitate supporting service users into clinic.

Work with service users to find the best plan of care for their needs.

Help service users stay mentally and physically well and work towards an optimum level of recovery including liaising with GPs and other health care providers.

To work closely with GPs, physical healthcare providers, partner organisations.

Complete Cardio metabolic assessments which may include taking basic observations and bloods etc (full training will be given)

Adhere to infection control procedures including sanitation of clinical areas and using PPE as required.

Exercise a degree of independence, initiative and judgement and consider different approaches when delivering tasks. Assess situations, identify and resolve potential problems, within own levels of competence.

Ensure the care experienced is positive and service user safety is maintained. Treat everyone eg, service users, their relatives, visitors, colleagues etc, with respect, dignity, courtesy and in accordance with the Trusts Values.

Contribute to the care planning process. Understand the principles underlying care planning eg, enabling the service user to reach an optimum level of functioning/ independence/recovery.

Implement care within well-established policies, procedures, protocols, guidelines and established clinical care plans. Organise a range of activities eg, facilitate meaningful activities for individuals or small groups.

Apply knowledge of longer term health conditions eg, Diabetes, Obesity, smoking cessation etc, providing advice and support.

Undertake the physical aspects of care eg, moving and handling of service users. Use items of equipment in delivering care eg, hoists, wheelchairs etc. (as appropriate to the role), ensuring appropriate instruction on their use has been received.

Assist in dealing with verbal/physical aggression using de-escalation techniques, this may include the need for restraint and/or seclusion (as appropriate to the role).

Participate in a flexible rostering arrangement providing 24-hour/7 day care (as appropriate to the role).

  • GCSE A-D or grade 3-9 in Maths and English or equivalent.
  • Apprenticeship level 3 or equivalent eg, RQF(regulated Qualification Framework)NVQ level 3 in Health & Social Care or equivalent.
