The specialist rehab and
recovery service in Calderdale have an exciting opportunity for a full time
Band 5 Staff Nurse. The role covers both the Community Based Rehab Team and the
Community Rehabilitation Unit. The role is supported by a Band 6 nurses, Band 4
Nursing Associate, and Band 3 Health Care Support Workers. The role is also
supported by Local Authority Residential Support Worker. The role forms an
important part of the Multi-Disciplinary Team.
The core function of the
service is to provide rehabilitation interventions to support people to return
to a level of function that they were prior to the onset of their illness. The
Service also looks for opportunities for integration into the community to
engage in meaningful activities and occupation around areas of interest. The
service looks at what accommodation might be appropriate to the individuals
needs, if there is any deficits identified. All individuals working in the team
will be committed to providing a holistic therapeutic approach, opportunities
and relationships that deliver a positive experiences. This will be achieved by
co production of care plans to achieve the maximum recovery status for that
individual and promoting self -management of symptoms of wellness
Calderdale Rehabilitation
Service incorporates Lyndhurst Hospital and a community-based rehabilitation
team. Both services are run from the Lyndhurst Hospital.
Main duties of the job
Lyndhurst Hospital is a 14
bedded level 1 Community Rehabilitation Unit and the community-based rehab team
offers up to 36 hours a day of rehabilitation in the community. Both services
forms part of the Rehab and recovery services in the Calderdale and Kirklees
BDU. The service predominately works with people suffering from psychosis who
have a specific rehab and recovery needs.
The Unit is led by a
multi-disciplinary management team, including occupational therapy, medical
input, and psychology with band 6 nurses taking a clinical leadership role. We
work closely with the community teams within Calderdale, to co-ordinate the individuals
care. Referrals are scrutinized by the MDT to establish outcomes and there is a
robust care review process.
The role acts as shift
co-ordinator for the service supported by management team. The role is to Maintain
a high standard of clinical work when carrying out nursing procedures. To be Responsible
for the safe custody of medication, including checking and administration. Ensure
compliance with our clinical governance framework, you will motivate
colleagues, champion service user safety and dignity and drive continuous
improvement. Co-produce care plan with a service user to achieve rehabilitation
outcomes and supporting the implementation of interventions following
assessments and feeding back progress to support the evaluation of care.
