The specialist rehab and
recovery service in Calderdale have and exciting opportunity for a full time
Band 3 Occupational Therapy Assistant/ Activity Co-Ordinator. The role covers
both the Community Based Rehab Team and the Community Rehabilitation Unit. The
role is supported by an excellent therapy team which is made up of two Senior
Occupational Therapist, a rotational band 5 Occupational Therapist and another
full time Occupational Therapy Assistant/ Activity Co-Ordinator. The Therapy
team form an important part of the Multi-Disciplinary Team.
The core function of the
service is to provide opportunities for integration into the community, and
accommodation appropriate to the individuals needs, using a Recovery Approach
supported by positive engagement and therapeutic interventions. All individuals
working in the team will be committed in providing a holistic therapeutic
approach, opportunities and relationships that delivers positive experiences.
This will be achieved by co production of care plans to achieve the maximum
recovery status for that individual and promoting self -management of symptoms and
wellness
Calderdale Rehabilitation
Service incorporates Lyndhurst Hospital and a community-based rehabilitation
team. Both services are run from the Lyndhurst Hospital.

Main duties of the job
The Band 3 Occupational Therapy
Assistant plays an essential role in the rehab and recovery service. The role
provides support to the registered therapy staff by supporting the
implementation of interventions following assessments and feeding back progress
to support the evaluation of care. The Occupational Therapy Assistant role also
undertakes some of the initial assessments which feeds into formulating the
care plans with an individual. The Occupational Therapy Assistant will look at
building individualised activity plans with a service user based around their
hobbies and interests. They will support with making initial contact with any
service/groups and support a service user to start building this into their
weekly routine. The role includes supporting rehab specific groups alongside
the Multi-disciplinary team and providing information and practical support
with individuals functional rehab.
Lyndhurst Hospital is a 14 bedded level 1 Community Rehabilitation Unit and the community-based rehab team offers up to 36 hours a day of rehabilitation in the community. Both services forms part of the Rehab and recovery services in the Calderdale and Kirklees BDU. The service predominately works with people suffering from psychosis who have a specific rehab and recovery need.
The Unit is led by a multi-disciplinary management team, including occupational therapy, medical input, and psychology with band 6 nurses taking a clinical leadership role.
