Kirklees Memory
Assessment Service are seeking to recruit a Band 6 nurse to work within their
team. This exciting opportunity is to work 33 hours per week with the Early
Onset Dementia Team. This post is based at Folly Hall in Huddersfield and the
role will cover both North and South Kirklees.
You would be
working with a well-established multidisciplinary team that supports staff to
develop both personally and professionally. We have a mix of Memory Nurses,
Advanced Nurse Practitioners, Social Care Professionals and Occupational
Therapists, with access to Dietetics and Psychology. A full comprehensive
induction will be provided and access to further training options.
Kirklees Memory Assessment Service has recently received
their third MSNAP accreditation, which is endorsed by the CQC. The service has
also received a Sustainable Mental Health Service Commendation.
You will need to be able to
work autonomously as well as within a team and be able to assess risks for
people with dementia.

Main duties of the job
The postholder
will work within the remit of the Band 6 job description which involves
undertaking initial assessments of service users, intervention work, risk
management and clinical decision making in line with leadership and working
collaboratively with partner organisations. Alongside this, ideally you will
hold a recordable Independent Nurse Prescribing Qualification to practice as a
Non-Medical Prescriber, although this is not essential and an experienced band
6 nurse is also encouraged to apply.
You would be
working with a well-established multidisciplinary team that supports staff to
develop both personally and professionally. We have a mix of Mental Health
Nurses, ANPs, Social Care professionals and OTs, with access to dietetics and
psychology. There are regular Nurse Prescriber network meetings and clinical
supervision available.
Experience of
working within the community would be advantageous.
Full UK driving
licence and use of own car is essential.
This role may be
eligible for skilled worker visa sponsorship, subject to the appointed
candidates salary meeting the minimum salary threshold of £38,700 per annum
under current UK government requirements.

Provide comprehensive mental health and social care assessments and provide care, treatment and educational interventions within the bounds of the agreed protocol and/or pathway within the service.
Review the efficacy of anti-dementia medication that has been prescribed for service users in line with the service pathway & protocol.
Act as care co-ordinator for service users as required.
Attend multidisciplinary meetings and care programme meetings as required.
Ensure that carers needs are assessed and appropriate interventions or onward referral is provided depending on need.
Communicate assessment outcomes sensitively to service users and relevant others. Inform the referring clinician of the assessment outcome and plan of care and treatment through provision of effective clinical reports.
