Hello, my name is Judith Megson, I am the team manager for the
older adult community mental health team (CMHT) in Kirklees.
We are seeking to recruit someone who matches our values
and who can support us to continue to provide high standards of assessment,
treatment and care to our service users, to enable them to live well in their
community.
This is an opportunity to work in a community team having a
strong focus on liaison activity and working collaboratively with service
users, colleagues and our older adult inpatient units to deliver timely
activity, interventions and discharge.
This is a dynamic and varied role where you will work as
part of a team and autonomously.
In return you will be supported and encouraged to develop
and grow as a clinical practitioner. There are multiple opportunities to
develop your skills and knowledge with team support and supervision.
The team take a strong focus on staff well-being and
actively seek ways in which we can look after each other.
The role will be across Kirklees utilising our community
bases at Beckside Court in Batley and Folly Hall in Huddersfield, working
Monday to Friday 9am-5pm. Flexible working options are available.
You will need to be a car driver and have the use of a
vehicle to undertake any necessary travel in connection with the duties.

Main duties of the job
A key element
of the role will be care coordination, supporting colleagues, duty worker and
prioritisation of workload in times of high clinical demand.
Undertake
assessments and review care plans of service users which promote engagement,
stability, recovery and risk management.
Manage a varied
caseload covering functional and organic mental health presentations.
Work
autonomously but contribute to the functioning of a multi-disciplinary team.
Work with
service users with both short-term interventions and severe and enduring mental
health needs to promote health and well-being.
Document care
delivery to a high standard in line with professional standards, local and
national guidance.
Recognises
situations that may be detrimental to the health and well-being of the service
user and initiates action that may be required.
Support
student education when allocated to the team and to contribute to the
preceptorship and education of newly qualified staff where appropriate.
Administration
of depot medication in the community.
Maintain
professional registration and demonstrate commitment to statutory codes of
practice and continued professional development requirements.

The post holder will be responsible and accountable for the assessment, planning, implementation and evaluation of skilled interventions to address identified needs and risk for allocated clients on a defined caseload. Works within a multi-disciplinary team as a member of a Core CMHT.
To work as a member of an integrated multi-disciplinary team providing comprehensive assessment of clients and their carers, planning, implementing and evaluating skilled interventions to address identified needs.
To have knowledge of current issues and research relevant to the legislative framework and systems in which mental health care is delivered e.g. CPA, MHA (1983).
To contribute to service development within the clinical governance structure and policies and procedures of the Trust. Adhering to NHS guidance outlined by the relevant statutory registration bodies.
Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.
To be competent in all aspects of care and risk management as defined within the framework of the care programme approach.
To work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients mental health and overall functioning, ensuring needs are met.
Management of enquiries and direct referrals of people in accordance with the Core C.M.H.T. Operational Policy and participates in the team duty rota. To participate in the team duty rota.
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
To be responsible for the safe custody, transportation and administration of medicines including I.M. preparations.(if appropriate to qualification)
Provide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity. To have an understanding of and participate in the clinical audit process.
To have an awareness of evidence based practice relevant to the specialist area and participate in the education of individuals and target group.
