This is an exciting opportunity
for you as an experienced practitioner with knowledge and experience of working
with people with mental health issues to work alongside the Core Community
Mental Health Team.
The Core Team is a large
multidisciplinary team of mental health professionals working with service
users with moderate and severe mental health needs supporting them with social,
occupational and/or psychological needs. We also support service users who have
long term mental illnesses and require medication monitoring and administration
via our Treatment Team clinics.
You will be providing a person centred
service and an opportunity to also work alongside other professionals in the
team to identify recovery focused interventions with our service users.
Ideally you will be an
experienced CPN who is able to work alongside the Core Team Treatment Team and
Medics to offer Nurse Led Community Clinics for our service users with chronic
long term mental illnesss that require on going monitoring and reviews of
their mental and physical well being.

Main duties of the job
You will need to demonstrate a good understanding of mental ill
health conditions e.g. psychosis, depression, anxiety and bi polar disorder and
how these can impact on daily activities and potential relapse features and risks
for service users. Also, to demonstrate an ability to motivate others, use
effective communication & interpersonal skills, and an understanding and
application of risk management.
You will be responsible for managing a caseload with a
certain level of autonomy and to carry out therapeutic interventions in
collaboration with service users to promote their independence and wellbeing. You
will be expected to work alongside staff in the Core Treatment Team to
administer depot and other medications. You will also be required to hold Nurse
led community clinics for service users with severe mental illness diagnoses
and work with the Core medics to ensure a seamless service is offered to service
users. Core also offers a range of other interventions including Psychological
therapy, OT and social care assessments and you will have the opportunity to
work alongside staff offering these interventions.
You will receive on going supervision and we are committed
to staff development and training in order to give a quality service to our
service users.
