A secondment opportunity has arisen for an experienced Mental Health Nurse to work within the Kirklees Memory Service based at Folly Hall in Huddersfield working 3 days a week for a 12 month period.

The Adults and Older People Mental Health Care Group are looking to recruit a Memory Nurse (Nurse Prescriber) for a part-time secondment, supporting both North and South Kirklees. The role encompasses the Memory Service and may also support the Early Onset Dementia Team. The service has recently received their third MSNAP accreditation with a Sustainable Mental Health Service Commendation, which is endorsed by the CQC.

The successful candidate will be expected to work autonomously as well as with the team and be able to assess and manage risks for people with dementia and their carers. The service places staff clinical and management supervision, appraisal, support and professional development as a high priority.

A full comprehensive induction will be provided along with access to further training options.

Main duties of the job

The postholder will work within the remit of the Band 6 job description which involves undertaking initial assessments of service users, intervention work, risk management and clinical decision making in line with leadership and working collaboratively with partner organisations. Alongside this, ideally you will hold a recordable Independent Nurse Prescribing Qualification to practice as a Non-Medical Prescriber, although this is not essential and an experienced band 6 nurse is also encouraged to apply.

You would be working with a well-established multidisciplinary team that supports staff to develop both personally and professionally. We have a mix of Mental Health Nurses, ANPs, Social Care professionals and OTs, with access to dietetics and psychology. There are regular Nurse Prescriber network meetings and clinical supervision available.

Experience of working within the community would be advantageous.

Full UK driving licence and use of own car is essential.

Provide comprehensive mental health and social care assessments and provide care, treatment and educational interventions within the bounds of the agreed protocol and/or pathway within the service.

Review the efficacy of anti-dementia medication that has been prescribed for service users in line with the service pathway & protocol.

Act as care co-ordinator for service users as required.

Attend multidisciplinary meetings and care programme meetings as required.

Ensure that carers needs are assessed and appropriate interventions or onward referral is provided depending on need.

Communicate assessment outcomes sensitively to service users and relevant others. Inform the referring clinician of the assessment outcome and plan of care and treatment through provision of effective clinical reports.

Promote health through provision of health education information and advice.

Following qualification as an independent non-medical prescriber, prescribe medication as an independent nurse prescriber within their own sphere of competence and the bounds of their approved prescribing status in accordance with the Trust non-medical prescribing policy.

Promote patient choice and understanding when communicating assessment outcomes or prescribing medicines, including provision of high quality information both written and verbal to support this.

Be responsible for ensuring the security and safe handling of FP10 prescription forms and to report any loss or theft as per Trust guidelines.
