Job summary

37.5 per week.

An opportunity has arisen within Kirklees Outreach Team (KOT) for an enthusiastic Band 6 practitioner on a full-time basis.

This is an excellent opportunity to join a supportive, forward-thinking team, committed to providing high quality care.

KOT is an older peoples crisis team that provides home-based treatment and intensive support for older people, whose mental health needs increase significantly either due to an acute episode, or relapse of an enduring mental health condition. The team aims to provide an alternative to hospital admission when safe and clinically viable to do so.

A referral to this team would be considered:

When people are experiencing the onset of relapse or severe mental distress, where a breakdown in coping mechanisms or a change in circumstances necessitates a rapid flexible and/or intensive response.

To provide brief interventions to augment/support existing care plans.

When hospital admission is being considered/a gatekeeping assessment is required.

To support early discharge/graded leave.

We are a collaborative and supportive service and genuinely appreciate the importance of clinical supervision and continuing professional development.

Main duties of the job

We are seeking individuals who are committed to delivering a high-quality service and who can work within a team approach, supporting service users Kirklees-wide.

Community Mental Health Practitioners within KOT are expected to:

Provide home based treatment as an alternative to hospital admission where clinically viable.

Reduce the length of inpatient stay.

Improve patient choice and experience.

Work for short, intensive periods with people who have acute or severe mental health problems.

Contribute to the delivery of planned care in the clients own home, accessing community resources.

The successful applicant will be able to demonstrate a preference of working within the field of older peoples mental health and an understanding of the challenges unique to that field.

The team are operational 8am 8pm 7 days per week and you would be expected to undertake 12-hour shifts across a 7 day rota.

Full UK driving licence is essential.

Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.

Functions as a member of a multi-disciplinary team.

Management of enquiries and direct referrals of people in accordance with Enhanced team operational Policy and participates in the team 24 hour duty rota including on call system (if applicable to the business continuity of the service).

Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
