We have a vacancy for a Band 6
community nurse within the Calderdale Memory Assessment Service.
This post provides an exciting
opportunity for a dynamic and innovative practitioner and the successful
candidate will work within an established Memory Assessment Service, based at
Laura Mitchell Health & Wellbeing Centre in Halifax, and will provide
advanced levels of autonomous nursing practices to assess, and investigate
within the guidelines of the memory pathway.
You will be working with a
well-established multidisciplinary team that supports staff to develop both
personally and professionally. We have a mix of Memory Nurses, Occupational
Therapists, Advanced Nurse Practitioners, Psychiatrists and Clinical Psychologists.
Afull comprehensive induction
will be provided and access to further training options.
The service places staff
clinical and management supervision, appraisal, support and professional
development as a high priority.

Main duties of the job
In
this role you will provide full cycle patient care by:
Assessing, stabilising, treating and managing
patients.
Independently and autonomously assess, plan,
implement and evaluate treatment & interventions and care for patients with
attention to physical, emotional, social, cultural, and mental health issues.
Be responsible for the administration and
supply of medication to patients to set criteria.
Providing support such as advice, guidance,
and training on professional nursing issues, and taking part in clinical
governance activities.
Work in a collaborative manner with others to
ensure seamless service delivery.
Participate in the use of reflective
practice, nursing research, audit, and clinical supervision.
Engage
with clinical supervision.

Effectively manage caseload of clients with

at every intervention with the person with dementia and their carer.
Identify and issues and signpost appropriate
service in relation to clients to administer or maintain their financial,
personal relationships, domestic, housing, spiritual, health, hygiene,
appearance and occupational needs.
Promote, facilitate and monitor concordance
with treatment.
Undertake risk assessment in accordance with
local guidelines. Identification of risk and acknowledgement with relevant
legislation and local policy on vulnerable adults.
Identify and highlight if awareness of risk of
abuse, failure to protect and harm to self and others in accordance with
relevant legislation and local policy on vulnerable adults.
Facilitate health promotion specific to the
needs of people with dementia and their carer.
Recognise
the health and social factors that precipitate crisis, work collaboratively
with the person with dementia and their care in the development of early
intervention plan in response to clearly identified warning signs.
Identify
and refer for resources to assist in the maintenance of independence.
Maintain
personal contact with the person with dementia and carers, working proactively
with professionals from own and other agencies, providing and receiving
complex, sensitive or contentious information.
Establish,
sustain and disengage working relationships with clients and their carers.
