37.5 Hours Per Week
We have a very exciting new
opportunity to recruit into a Band 5 role (37.5 hours - temporary contract till
31s March 2026) at the North Kirklees EIP team. We are looking for a self motivated,
enthusiastic individual who has a desire to work to improve life chances of
people who may be experiencing their first episode in psychosis.
This can take
a lot of patience, requiring keen skills in creative engagement. Part of the
role will involve health promotion; improving the physical heath of this group
of service users as well as promoting the role of the insight team to capture
appropriate referrals.
The team covers the North
Kirklees locality, working with people aged 14 to 65 and stretches across adult
and childrens services. The team forms part of the community service line and
Enhanced Pathway within the North Kirklees BDU.
We will consider applications
from a newly qualified Band 5 who wish to transition into the role with a fully
supported integration and coaching process.

Main duties of the job
The post holder will be responsible for coordinating the care of approx.
15 service users who have experienced first episode of psychosis or are at
ultra-high risk of developing it.
Providing practical
social, emotional and vocational support for individuals aged 14-65 following a
diagnosis of a psychotic illness. To enable service users to exercise informed
choices about their lives where possible and to enhance their social skills and
ability to participate in the community.
Provide a recovery focused service to
individuals to ensure they are aware of their rights and responsibilities
within the project and society. Support individuals through a tailored support
programme with a view to move-on to community engagement and vocational
opportunities.
Supporting individuals through a range of activities, on an
off-site and peripatetic basis. Facilitating the use of local community
services. Identifying and meeting the needs of individuals to enable them to
maintain their health and well being through engaging individuals in social
inclusion groups as well as the cardio-metabolic assessment clinic. Providing
information, advice, and guidance as appropriate.
Assisting individuals in identifying
areas of support and in line with their Care & treatment plan.

defined caseload. Works within a multi-disciplinary team as a member of an Early
Intervention in Psychosis Team.

comprehensive assessment of clients and their carers, planning, implementing,
and evaluating skilled interventions to address identified needs.

knowledge of current issues and research relevant to the legislative

contribute to service development within the clinical governance structure
and policies and procedures of the Trust. Adhering to NHS guidance

the person and carers. Implements the planned programme of care to ensure
a high standard is achieved. Reviews the effectiveness of the care

risk management as defined within the framework of the care programme
approach.
To work collaboratively with clients,
