Job summary
An exciting opportunity has arisen for a suitably qualified Social Worker, CPN or OT, at band 5 level, to work in the South Kirklees Core Team: a well-established Community Mental Health Team for adults. The post offers a wonderfully diverse working role encompassing working alongside other professionals within the Core team and seeking to create supportive and recovery focused care plans with service users. The Core Team, in conjunction with the Trauma Informed Personality Disorder Pathway, aims to provide trauma-informed care to individuals with complex needs. The post-holder will have the opportunity to work within a friendly, fully integrated multi-disciplinary team who are dedicated to developing better services guided by trauma-informed principles of care.
Main duties of the job
We are looking for an enthusiastic individual who is flexible and creative in response to service pressures and patient needs. The role will involve a wide range of activities including holding a caseload, working alongside other professionals such as Psychiatry, OT, Psychological therapists and Social Workers to co produce recovery focused care plans. The Core Team offer a range of short term interventions such as, helping service users to gain an understanding of emotional regulations, dealing with distress, crisis and contingency planning as well as identifying potential OT interventions or addressing social care issues, including Care Act Assessments. The Core Team also support service users with more chronic mental health needs and run Community Clinics and a Treatment Team offering medications and advice from our Psychiatrists. The Core Team also work with other local community organisations, such as the Recovery College, to aid a supportive discharge from secondary mental health services into primary care, including GPs.
You will be expected to work with a caseload and be able to work autonomously and as part of a large team. Regular supervision is available as well as a commitment to personal and professional development.
