Job summary
These roles are an exciting
opportunity to work within the local CAMHS mental health team across the
Primary Care Network (PCN) in Kirklees.
You will work with Primary
Care, schools, Core CAMHS, Kirklees Keep in Mind and Eating Disorders colleagues
to deliver high quality care in the local PCNs forming a vital link between
secondary and primary care.
The aim of this new service is
to provide early intervention, assessment and treatment to reduce referrals
into Core CAMHS and reduce inappropriate referrals to A&E. This is in line
with the NHS plan to ensure children and young people are seen at the right
time, in the right place for the right care.
The primary function of the
service in practice is to safely and effectively provide triage, assessment,
formulation, signposting and timely discharge.
The post holder may engage in
brief therapeutic work with the young person referred, refer them to another
service/agency or advise them of other sources of support that they could
access. The post holder will build and maintain effective links with all
disciplines within the PCNs and will work as an integrated part of the PCN MDT.
This work will focus
on children and young people who are identified as requiring mental health
support to access treatment and improve their wellbeing.
Main duties of the job
We welcome
professional registrants in Nursing, Social Work, and Occupational Therapy to
apply. There is opportunity for Nurses to complete their non-medical
prescribing and aspirations to become a trainee ACP are welcomed.
