The Intensive Support Team
Calderdale (ISTC) are seeking an enthusiastic and autonomous Band 6
practitioner on a full-time basis.
This is an excellent opportunity
to join a Dynamic and dedicated team committed to providing high quality care.
ISTC is an older peoples crisis
team that provides home-based treatment and intensive support for people, whose
mental health needs have increased significantly, either due to an acute
episode, or relapse of an enduring mental health condition. The team aims to
provide an alternative to hospital admission when safe and clinically viable to
do so.
A referral to this team would be
considered:
When people are experiencing
the onset of relapse or severe mental distress, where a breakdown in coping
mechanisms or a change in circumstances necessitates a rapid flexible and/or
intensive response
To provide brief interventions
to augment/support existing care plans.
When hospital admission is
being considered/a gate keeping assessment is required.
To support early
discharge/graded leave.

Main duties of the job
ISTC are seeking an individual who is committed to delivering a
high-quality service.
The successful applicant will demonstrate strong clinical and leadership
qualities, working as part of an MDT approach, supporting service users across
Calderdale.
Community Mental Health Practitioners within ISTC are expected to:
Provide home based treatment as an alternative to hospital admission
where clinically viable.
Reduce the length of inpatient stay.
Improve patient choice and experience.
Work for short, intensive periods with people who have acute or severe
mental health problems.
Contribute to the delivery of planned care in the service users own
home, accessing community resources.
The successful applicant will have a passion for working within the field
of older peoples mental health, as well as experience of working within a
multi-disciplinary mental health setting.
The teamwork 8am - 8pm 7 days per week and you would be expected to
undertake 12-hour shifts across a 7-day rota.
Full UK driving licence is essential.

Has
continuing responsibility for the provision of skilled interventions to people
with mental health problems in the community. Works within a multi-disciplinary
team as part of the intensive support team.
Management
of team caseload in line with C.P.A. procedures and risk management protocols.
Provision
of leadership to the Kirklees Outreach Team in respect of psycho-social
interventions to people and their carers in line with national and local
practice standards e.g. NIC.
Supervision and support of other team members.
Participation in service development and clinical
governance structures.
Assesses
needs and identifies problems relevant to the care of people referred.
Devises a plan of care in partnership with the person
and carers.
Implements the planned programme of care to ensure a
high standard is achieved.
Reviews the effectiveness of the care provided and
where appropriate initiates any action.
Advises and supports the person and carers to promote
health and well-being and to prevent illness.
Recognises situations that may be detrimental to the
health and well-being of the person and initiates action that may be
required.
Establishes and maintains satisfactory communication
with all agencies involved in the persons care in both hospital and
community.
Functions as a member of a multi-disciplinary team.
Management of enquiries and direct referrals of people
in accordance with Enhanced team operational Policy and participates in
the team 12 hour duty rota
Maintain professional registration and demonstrate
commitment to statutory codes of practice and continued professional
development requirements.
Responsible for acting up in the absence of the team

Maintains
accurate patient care records in accordance with professional codes of conduct
and trust procedures and ensures records are made available when required by
the appropriate agency.

safe carriage, storage and administration of drugs prescribed (registered
nurses only).
